Labs
It is advised that you check with your insurance company for preferred laboratory facilities. You may have the option to directly view lab results on the laboratory website.

Office Policies
- As of January 1, 2012, patients will be responsible for a $50 No Show Fee for cancelling or rescheduling their appointment within 24 hours of their appointment time.
- In general, if you have not seen your physician in over six months, you will need to come in for a follow-up appointment prior to further prescription refills.
- We ask that all diabetic patients bring in either
1) their glucometer, or
2) their home blood glucose monitoring logsheets, to all appointments. This ensures efficient appointments and better care.
